The manufacturer of the thrill ride that malfunctioned and killed one and injured several on July 26 said the fatal incident was caused by "excessive corrosion".
The Fireball ride, which swings and spins its passengers back and forth, was built in 1998 by Dutch manufacturer KMG, The Associated Press reported.
Corrosion on the interior of the support beam reduced the beam's thickness, which led to the accident at the fair, KMG International said in a statement released Sunday.
KMG said it will collaborate with "industry safety experts to develop an inspection protocol" to prevent similar incidents in the future. "On the Afterburner in OH we did the maintenance until a few years ago", Kroon said at the time.
Ohio Department of Agriculture records showed passing marks on inspections for Fire Ball on about three dozen items, including possible cracks, brakes, proper assembly and installation. Jarrell had recently enlisted with the U.S. Marine Corps and was supposed to attend basic training next year.

The company had ordered fair and carnival operators throughout the country to stop using the Fire Ball ride and others like it following the accident at the Ohio State Fair. The fair's last day was Sunday.
Following the ride's malfunction, officials with the Ohio State Fair shut down other rides.
Four of the injured remained hospitalized last week, including an 18-year-old woman in a coma, and Jarrell's 18-year-old girlfriend. The company also conducted a metallurgical inspection of the ride.
Jarrell's family is pursuing a wrongful death lawsuit.
